Making a referral to Beacon Support Group

We welcome referrals for adults aged 18–65 with mental health needs across Crawley and the wider West Sussex area, including planned hospital discharges and step-downs from higher-intensity support.

Who can refer

  • Local authority commissioners and brokerage teams
  • Social workers and care coordinators
  • Community mental health teams and CPNs
  • Hospital discharge and inpatient teams
  • Housing providers and supported housing teams
  • Advocates, families and self-referrals

Information that helps us

You do not need everything below to make contact — send what you have and we will work through the rest with you.

  • The person's name, age and current living situation
  • A summary of mental health needs, diagnosis and current support
  • Any existing needs assessment, support plan or risk assessment
  • Current risks, relapse indicators and crisis history
  • Medication and physical health needs
  • Accommodation requirements (single or shared, accessibility)
  • Proposed hours of support and funding arrangements
  • Key professionals involved and their contact details

What happens after you refer

    Get in touch

    Call or email us with an outline of the referral. We will confirm whether we can progress to assessment.

    Share information

    Send the assessments and risk information you hold so we can understand needs fully and safely.

  3. 3

    Assessment

    We meet the person, and the professionals involved, to assess needs, strengths, aspirations and risks.

  4. 4

    Proposal & agreement

    We confirm whether we can meet the need, the support hours proposed and the costs involved.

  5. 5

    Planned transition

    Support plans and risk assessments are agreed, staff are matched and introductions take place before the move.

We will always be honest about whether we can safely meet someone's needs. If we cannot, we will say so promptly so that alternative support can be found without delay.
